Preschool | Summer Camp Forms
The same forms need to be filled out for both preschool classes and summer camp. Current students/parents do not need to resubmit these forms for summer camp if they have already done so for preschool.
In addition to the forms below, a current WELLNESS EXAM & IMMUNIZATION RECORD must also be submitted. You can obtain these records from your childs pediatrician or request them to send these documents directly to Lyme Nursery School.
